Referral Process

    1. If you suspect your child has a disability, write a letter to the school district with a request for assessment. Provide your child's name, contact information, and your concerns.
    2. Drop of your letter to Lewis Early Learning Academy at 2230 Lomitas Avenue or email your letter to jplack@srcs.k12.ca.us
    3. If all parties agree to an evaluation, you will be sent an intake packet. Complete all paperwork and return it to Lewis Early Learning Academy.
    4. Register your child with the school district.
    5. Evaluating staff will contact you to schedule a school based assessment, home assessment, or both.
    6. An IEP meeting will be held to determine whether your child qualifies for special educaiton. If so, an IEP will be developed.
